About The Position

Based in Cerritos, CA, we proudly provide specialized in-home medical care to elderly and chronically ill patients throughout Los Angeles and Orange County. Do you have a passion for engaging with people and improving their health? Prudential MD offers comprehensive mobile medical services tailored to meet patients where they feel most comfortable. We specialize in the care and management of geriatric populations and deliver exceptional care to private homes, senior living communities, and assisted living facilities. We are currently seeking a Per Diem/Part-Time Nurse Practitioner or Physician Assistant to join our mobile care team. In this role, you will provide in-home health evaluations, build meaningful relationships with patients, and help bridge gaps in care. The typical visit lasts about 45 minutes, allowing time to connect one-on-one, review medications and medical history, answer questions, and complete a physical evaluation.

Requirements

• Active NP or PA license in California
• Valid driver’s license and reliable transportation
• Strong communication and interpersonal skills
• Ability to work independently

Responsibilities

• Provide compassionate, patient-centered in-home health evaluations
• Deliver comprehensive assessments, including medication review, medical history, and physical exam
• Engage with patients in their homes, assisted living, and senior community settings
• Build trusted relationships to support better health outcomes
• Travel locally within Los Angeles and Orange County
• Maintain accurate clinical documentation
• Communicate effectively with the care team as needed
