This position will work remote & in the field about 75% of the time in San Diego, CA & the surrounding areas. We are seeking an experienced and motivated Advanced Practice Clinician (APC) to join our healthcare team based in Riverside, CA. The APC will be responsible for providing high-quality patient care, collaborating with physicians and other healthcare professionals, and ensuring that all clinical practices are in line with the latest medical standards. This specific role involves caring for our highest-risk patients, in collaboration with our Regional Medical Director.

Requirements

  • Completion of an accredited Advanced Practice Clinician program required.
  • CA Registered Nurse (RN) required.
  • 3-5 years of related experience required
  • Travel 75+% of the time in the assigned area- Must have a valid driver’s license, automobile insurance and reliable transportation.
  • Good communication and interpersonal skills
  • Strong knowledge of clinical practices and procedures.
  • Ability to independently diagnose and treat common chronic diseases.
  • Ability to analyze complex patient data and make informed clinical decisions.
  • Strong problem-solving skills to address patient care challenges.
  • Ability to work collaboratively with a multidisciplinary healthcare team.
  • Strong interpersonal skills to build effective relationships with patients and colleagues.
  • Proficiency in electronic health record (EHR) systems and other clinical software.
  • Ability to use technology to enhance patient care and documentation.
  • Ability to manage a busy caseload and prioritize tasks effectively.
  • Strong organizational skills to ensure timely and efficient patient care.
  • Ability to maintain accurate and detailed patient records.
  • Meticulous in following clinical protocols and procedures.

Responsibilities

  • Conduct comprehensive patient assessments, including medical history, physical exams, and diagnostic tests.
  • Develop and implement treatment plans, including prescribing medications and therapies.
  • Provide patient education and counseling on health maintenance, disease prevention, treatment options, and goals of care.
  • Work closely with physicians, nurses, and other healthcare professionals to ensure coordinated and comprehensive patient care.
  • Participate in interdisciplinary team meetings and case conferences to discuss patient care and treatment plans.
  • Monitor and evaluate patient outcomes to identify areas for improvement in care delivery.
  • Participate in quality improvement initiatives and research projects to enhance clinical practices and patient outcomes.
  • Maintain accurate and detailed patient records, including medical histories, treatment plans, and progress notes.
  • Ensure compliance with all regulatory and documentation standards.
  • Stay current with advancements in medical knowledge and clinical practices through ongoing education and training.
  • Actively support the achievement of SCAN’s Vision and Goals.
  • Other duties as assigned.
