About The Position

Horizon Health in Wessington Springs, South Dakota, is seeking a full-time Nurse Practitioner or Physician Assistant to provide direct patient care. The role coordinates all aspects of primary care and includes emergency room and hospital coverage with weekday and weekend rotation. The successful candidate will provide patient-centered care in a collaborative, team-oriented environment focused on individual patient needs and community health and wellness. The position offers a base salary with additional compensation for ER call. Candidates must be licensed, or able to obtain licensure, in South Dakota. Certification from an accredited organization is required before employment. ACLS, ATLS, and PALS certifications are required, or the candidate must be willing to obtain them upon hire. Requirements

Must be licensed or able to successfully obtain licensure in the State of South Dakota.
Certification from an accredited organization is required or must be obtained before employment.
ACLS, ATLS, and PALS certifications are required, or the candidate must be willing to obtain them upon hire.

Responsibilities

Provide primary health care to patients according to established evidence-based clinical practice guidelines.
Lead care teams in delivering high-quality healthcare in accordance with Horizon Health’s Quality Improvement Plan and meeting established goals and benchmarks.
Manage long-term care for acutely and chronically ill patients, including care coordination and development of care plans and self-management plans.
Provide emergency room and hospital coverage, including weekday and weekend rotation.
