About The Position

Signify Health, part of CVS Health®, is seeking a Full-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with patients in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The evaluations are comprehensive, including medication and medical history review, physical evaluation, and potentially diagnostic tests. Clinicians use an iPad with an intuitive workflow and are not required to prescribe medication, order lab tests, or alter current treatment regimens. The primary workplace is the patient's private residence, which can vary in conditions, and candidates must be comfortable working in diverse home environments that may include pets, secondhand smoke, or other airborne allergens.

Requirements

Active, unrestricted license(s) in coverage area(s).
Board certification (required).
Able to cover the following primary locations: Providence and surrounding area.

Multi-state licenses (preferred).

Responsibilities

Conduct In-Home Health Evaluations.
Engage with patients in their homes to bridge gaps in care.
Build trusted relationships to improve health outcomes.
Conduct medication and medical history reviews.
Perform physical evaluations.
Conduct additional diagnostic tests if ordered by the patient's health plan (e.g., diabetic eye exams, spirometry).
Use an iPad and an intuitive clinical workflow for each evaluation.
Fulfill the requirements of the specified role as a clinician colleague with Signify Health.
Be flexible to travel locally and within licensed states, as needed.
Conduct virtual visits as needed.
Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.).
