Pulmonary / Ventilator Experience Preferred. We have flexible employment opportunities across multiple scheduling options. Opportunities include: Full Time / Part Time / PRN. Impact Health PC is a leading advanced practitioner group serving skilled nursing facilities throughout the Northeast, dedicated to prioritizing patient care. We specialize in Medical Primary Care, Psychiatric Services, Wound Care, and Palliative Care. This position primarily supports a skilled nursing facility with a medically complex patient population, including a ventilator unit. Experience caring for patients with pulmonary conditions or ventilator-dependent residents is strongly preferred but not required. Requirements
- Active Nurse Practitioner (NP) or Physician Assistant (PA) license
- Minimum of two years of post-certification clinical experience
- Proficiency with EHRs and clinical technology
- Strong communication and collaboration skills
Nice To Haves
- Experience in skilled nursing facilities, hospitalist roles, internal medicine, emergency medicine, ICU, or pulmonary medicine
- Experience caring for patients with pulmonary conditions or ventilator-dependent residents
- Experience caring for medically complex or ventilator-dependent patients is a plus
Responsibilities
- Deliver person-centered medical care using evidence-based practices
- Manage acute and chronic medical conditions in the skilled nursing setting
- Provide care for medically complex patients, including residents with chronic pulmonary conditions and ventilator needs, as appropriate
- Collaborate with nursing, respiratory therapy, and interdisciplinary facility teams
- Conduct assessments, develop treatment plans, and manage follow-up care
- Educate patients, families, and staff
- Complete timely documentation (within 24 hours)
- Ensure compliance with state regulations in skilled nursing facilities
