About The Position

This job is an integral part of the care delivery system, working to provide a healing environment by placing customers first. The incumbent provides primary healthcare services for non-acute or chronic health conditions in specific populations in accordance with applicable scope and standards of practice and with the policies, values, and mission of the organization within the CRNP scope of practice. Work is performed in a physician office or offices dedicated to Primary Care, with weekend and on-call coverage on a rotational basis, as required. The specific schedule includes Wed-am/pm/eve, Fri am/pm/eve and Sat 8am-2pm (Vandergrift).

Requirements

  • Master's degree MSN from the practicing State Board of Nursing Approved Master's Program (for Nurse Practitioner)
  • National Board Certification for Nurse Practitioners through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), National Certification Corporation (NCC), or Pediatric Nursing Certification Board (PNCB) (for Nurse Practitioner)
  • Practicing-state licensure as a Nurse Practitioner (for Nurse Practitioner)
  • Prescriptive Authority from the practicing state (Within 60 days of hire) (for Nurse Practitioner)
  • Drug Enforcement Agency Licensure (Within 60 days of hire) (for Nurse Practitioner)
  • CPR – American Heart Association (for Nurse Practitioner)
  • Act 34 Criminal Background Clearance Certificate (for Nurse Practitioner)
  • Act 33 Child Abuse Clearance Certificate (for Nurse Practitioner)
  • Act 73 FBI Fingerprinting Criminal Background Clearance Certificate (for Nurse Practitioner)
  • Master’s degree in Physician Assistant Studies (for Physician Assistant)
  • Current PA Physician Assistant License (for Physician Assistant)
  • PA-C or board eligible. Ongoing successful recertification (NCCPA) as a PA-C (for Physician Assistant)
  • Drug Enforcement Agency Licensure (Within 60 days of hire) (for Physician Assistant)

  • Four to ten years related experience and/or training; or equivalent combination of education and experience (as a substitution for degree requirements)

Responsibilities

  • Provides appropriate patient care to include recording patient medical history and physical assessment, developing therapeutic care plan, ordering diagnostic tests, interpreting test results, providing accurate medical record documentation.
  • Communicates and coordinates patient care with patient, family, community health services and interdisciplinary team.
  • Involves attending physician regarding plan of care and involves physician in clinical decision making of any issues that are outside the CRNP's knowledge base or scope of practice.
  • Provides monitoring and continuity of care and coordinates periodic evaluation by physicians as defined in the written agreement between the CRNP and the supervising physician(s).
  • Prepares written prescription orders for drugs and controlled substances, administers medication and injections, sutures wounds and provides wound care.
  • Educates patients and family members regarding health and preventive medicine, medications prescribed, treatment instructions and recommends community resources as appropriate.
  • Triages patient telephone calls, provides monitoring and continuity of care.
  • Participates, develops, and implements performance and/or quality improvement initiatives.
  • Ensures compliance with all regulatory agencies.
  • May perform other duties such as manage outpatient concerns and any other duty specific to specialty.
  • Other duties as assigned or requested.
