About The Position

Akicita Federal is hiring a Nurse Practitioner (NP) or Physician Assistant (PA-C) to deliver comprehensive outpatient primary care to the Oglala Lakota Nation through the Indian Health Service's Pine Ridge Service Unit in Pine Ridge, SD. This is a high-impact role serving one of the most underserved communities in the United States, where access to quality healthcare is a matter of life and safety. You will be part of a clinical team providing same-day appointment care at Pine Ridge Hospital, helping to close critical gaps in health access for a rural Native American population.

Requirements

  • Current, active national board certification as a Nurse Practitioner (NP) or Physician Assistant (PA-C)
  • Certification as an adult-focused or family Nurse Practitioner, or equivalent PA-C scope applicable to outpatient adult primary care
  • Active, unrestricted state licensure to practice as an NP or PA-C (South Dakota licensure or eligibility required)
  • Current DEA registration and prescriptive authority
  • Ability to provide direct patient care in an outpatient primary care clinic environment
  • Compliance with IHS credentialing and privileging requirements, including background investigation clearance as required by the Indian Health Service
  • Current Basic Life Support (BLS) certification

Nice To Haves

  • Prior clinical experience serving Native American, Alaska Native, or other federally recognized tribal communities
  • Experience practicing in a critical access, rural, or frontier healthcare setting where resource constraints require independent clinical decision-making
  • Familiarity with Indian Health Service systems, policies, or the Indian Health Manual (IHM)
  • Experience working with populations facing high rates of chronic disease, health disparities, or socioeconomic barriers to care
  • Comfort with high patient volumes in a same-day or walk-in outpatient clinic model

Responsibilities

  • Diagnose and treat acute and chronic conditions for adult patients in a fast-paced outpatient same-day clinic setting
  • Provide comprehensive patient assessments, physical examinations, and individualized care plans aligned with IHS clinical standards
  • Prescribe medications and order diagnostic tests, interpreting results to guide treatment decisions
  • Coordinate patient care with onsite physicians, specialists, and ancillary staff to ensure continuity of care and appropriate referrals
  • Document all clinical encounters accurately and timely in the Electronic Health Record (EHR) in accordance with PRSU policies and procedures
  • Respond to acute and urgent patient needs within the clinic, applying sound clinical judgment in a setting where higher-level care may be 35+ minutes away by air
  • Follow all Pine Ridge Service Unit facility protocols, compliance requirements, and quality standards to support safe, patient-centered care
