About The Position

Atlas Medical is expanding its Home Health & Hospice Provider Team and is seeking a Nurse Practitioner or Physician Assistant to provide mobile, on-site primary care to patients living in assisted living and other senior communities in Tucson, Arizona. The provider will independently manage complex healthcare situations, including patients with multiple co-morbid conditions, and educate patients and families about self-care, disease management, and end-of-life care. The role involves regular collaboration with facility staff and Atlas Patient Care support teams, while following Atlas quality measures and maintaining confidentiality.

The position is a contract role with full-time hours. Compensation is $70 per hour for up to six months, followed by a fee-for-service arrangement. Requirements

• At least one year of primary care experience.
• Valid licensure as a Nurse Practitioner or Physician Assistant in the state of practice.
• Ability and desire to work independently and drive between communities.
• Effective verbal and written communication skills.
• Time management skills.
• Valid Arizona Fingerprint Clearance Card.
• Current CPR certification.
• Proof of a negative TB test.

Responsibilities

• Deliver mobile, on-site primary care to patients residing in assisted living and other senior communities.
• Independently manage complex healthcare scenarios, including patients with multiple co-morbid conditions.
