About The Position

Impact Health PC is hiring a Nurse Practitioner or Physician Assistant in the Milford Area, CT. We are a leading advanced practitioner group in skilled nursing facilities across the Northeast, dedicated to prioritizing patient care. We specialize in Medical, Psychiatric Services, Wound Care, and Palliative Care. This role offers flexible employment opportunities with a Monday through Friday schedule, no nights, weekends, or holidays. Requirements

• Active license as a Nurse Practitioner (NP) or Physician Assistant (PA)
• Minimum 2 years of post-certification experience
• Proficient with EHRs and clinical technology
• Strong communicator with a collaborative mindset

Nice To Haves

• Experience in SNFs, hospitalist roles, emergency departments, or ICU preferred

Responsibilities

• Deliver person-centered care using evidence-based practices
• Collaborate with interdisciplinary facility teams
• Conduct assessments, develop treatment plans, and manage follow-up care
• Educate patients, families, and staff
• Complete timely documentation (within 24 hours)
• Ensure compliance with state regulations in SNF settings
