The Advanced Practice Provider (APP) contributes to the clinical, educational, and research missions of the organization. This role may involve providing clinical care in inpatient or outpatient settings, telehealth, and triage. The APP conducts regular health assessments, diagnoses, and manages acute and chronic conditions, formulating comprehensive plans that include treatment, psychosocial support, health education, and counseling. Collaboration with other health professionals is key for assessing and managing health problems, ensuring continuity of care, and coordinating community services. The APP also contributes to the development, maintenance, evaluation, and evolution of the healthcare delivery system. Essential duties include providing quality care for patients and families, managing chronic and acute conditions, and promoting health. This involves performing and documenting history and physical exams, gathering and reviewing medical data, communicating with referring providers, and developing and implementing patient care plans. The APP monitors and documents patient progress, revises care plans as needed, and interprets test results. They identify and intervene appropriately with problems requiring specialty referral or escalation. Additionally, the APP acts as a consultant, presents educational programs to community groups, and contributes to the organization's education and research mission through participation in projects, presentations, professional organizations, precepting, and other scholarly activities. The APP functions as a role model, encouraging professional behaviors, and represents advanced practice on organizational committees. Compliance with all applicable policies, laws, rules, and regulations, including those from the Colorado Medical Board, Joint Commission, Medicare, and Medicaid, is required. Maintaining current licenses, board certifications, and BLS/ACLS/PALS certifications is also essential.
