About The Position

Join a growing team of geriatrics providers at Hartford Healthcare and make a meaningful impact in the lives of geriatric patients! We are seeking a full-time Nurse Practitioner or Physician Assistant for our REACH (Resource for Elderly and Care at Home) Program, delivering compassionate primary care in the comfort of geriatric patients’ homes and assisted living facilities. Requirements

• Unrestricted State of Connecticut Medical License (or eligible)
• APRN (Family, Adult/Geri Nurse Practitioner) or PA training
• At least two years of post-graduate experience as a Nurse Practitioners (APRN) or Physician Assistants (PA).
• Primary Care/Geriatric Medicine experience highly preferred.
• New grads with relevant nursing experience are welcome to apply!

Responsibilities

• Perform routine physical examinations.
• Formulate medical, educational, and psychosocial plans to treat patients.
• Develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.
• Provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols.
• Identify problems outside of their scope of practice and refer appropriately to physicians.
