Nurse Practitioner or Physician Assistant
About The Position
Amaze Health is seeking a compassionate, skilled Nurse Practitioner or Physician Assistant to join its virtual care team. Amaze provides concierge-style virtual care for sudden illnesses, chronic conditions, and mental health needs, with an emphasis on helping patients feel heard, supported, and empowered. The role suits advanced practice providers who value empathy, continuous learning, and patient-first care in a flexible, on-demand environment. This is a virtual position. Applicants must live in Arizona, Colorado, Florida, Ohio, or Texas. The schedule is four 10-hour days with rotating weekends. The currently available shift is 12 p.m. to 10 p.m. MST. Pay is $55–$70 per hour.
Requirements
- Active, unrestricted advanced practice provider license.
- Nurse Practitioners must hold a compact nursing license and be certified as a Family Nurse Practitioner (FNP).
- At least five years of independent APP practice managing patient care in a family, primary, or urgent care setting.
- Experience delivering virtual care using digital tools and platforms, including urgent virtual visits, chronic care follow-up, and mental health conversations in a patient-facing care setting.
- Experience coordinating care plans with physicians and the broader care team.
- Strong communication and digital literacy skills.
- Must live in Arizona, Colorado, Florida, Ohio, or Texas.

Responsibilities
- Ask thoughtful questions through video or secure chat to identify the underlying causes of illness, injury, or medical conditions.
- Use clinical expertise, patient history, and remote diagnostic tools to assess and diagnose patients in a virtual setting.
- Order, interpret, and follow up on lab tests, using digital platforms to personalize and coordinate care plans.
- Write prescriptions electronically and explain their proper use, administration, and potential side effects through virtual or written communication.
- Collaborate with physicians, other advanced practice providers, and the broader care team to coordinate patient care.
- Develop creative, patient-centered responses to challenges in virtual care.
