About The Position

This position is part of the medical clinical team and is responsible for providing primary care and/or walk-in care services. This includes assessing and managing patients in the clinic according to established standards of practice. The role involves making independent decisions about advanced practice clinical needs and medical decisions based on the scope of practice. Providers in Oklahoma facilities will practice under a collaborative agreement with one or more physicians.

Requirements

  • If working in Kansas, valid Kansas Nurse Practitioner or Physician Assistant license.
  • If working in Oklahoma or Columbus WIC, valid Oklahoma Nurse Practitioner or Physician Assistant license with prescriptive authority required. Oklahoma license may be required by other APPs, depending on location and practice.
  • If Nurse Practitioner, valid Kansas Registered Nurse license or multistate license in nurse’s resident state. An Oklahoma Registered Nurse license may be required, if no multistate license.
  • Graduate of an accredited Advanced Nurse Practitioner or Physician Assistant program.
  • Current board certification.
  • If position/practice requires, must obtain DOT Certification within 30 days of hire and recertify every 10 years.
  • ACLS required for Pleasanton providers and WIC providers in Fort Scott and Pittsburg.
  • Must work a minimum of one (1) available shift every thirty (30) days to remain eligible for a PRN position. If no shifts are available in a thirty (30) period, employee will remain eligible.
  • Social and cultural sensitivity appropriate to ethnically and economically diverse patient-and employee-base.
  • Demonstrate knowledge of the rationale of appropriate patient care.
  • Communicates through appropriate channels. Use proper chain of command for patient complaints.
  • Ability to handle emergency situations calmly and effectively.
  • Must be computer literate and able to navigate the Electronic Health Record.
  • Must be able to maintain good inter-personal relationship with co-workers and other members of the health care team and the organization.
  • Provide customer service in accordance to the organization’s mission.
  • Be courteous and respectful when interacting with patients and family members.
  • Maintain patient confidentiality in accordance to organization’s policy and procedure and HIPAA requirements.
  • Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals.

Responsibilities

  • Develops and manages the medical plan of care for patients based upon a jointly developed and signed agreement between the mid-level provider and supervising physician.
  • Provides complete, comprehensive outpatient care for patients, including health examinations and treatment of medical conditions, with follow-up of detected problems.
  • Manages acute and chronic illnesses and routine care through care plans coordinated with the patient's behavioral health provider, when appropriate.
  • Makes appropriate referrals to internal disciplines of CHC/SEK and/or external resources/agencies for additional services.
  • Prescribes medications for patients for treatment of conditions, as indicated.
  • Provides clinical consultation to other providers within CHC/SEK as needed.
  • Ensures accurate, timely, and clear medical record documentation as defined by the Electronic Health Record Completion Policy.
  • Observes clinic protocols, guidelines, and scope of practice as determined by professional regulatory agencies.
  • Provides appropriate follow-up with clinical support staff, including telephone/written correspondence (e.g., informing patients of test results).
  • Communicates clearly in writing and orally with patients/guardians and employees.
  • Participates in an on-call system for after-hours care for the CHC/SEK inpatient population, as assigned.
  • Participates in quality improvement activities.
  • Supervises healthcare students as assigned.
  • Floats to alternate CHC/SEK facilities as needed to ensure appropriate access to care.
  • Performs other duties as assigned.
