About The Position

Signify Health, part of CVS Health®, is seeking a Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with individuals in their homes to bridge gaps in care and build trusted relationships. The clinician will conduct approximately 45-minute visits, including medication and medical history review, physical evaluation, and potentially diagnostic tests. The role utilizes an iPad with an intuitive clinical workflow and does not require prescribing medication, ordering lab tests, or altering current treatment regimens. The primary workplace is the private residence of health plan members, which can be diverse and may contain pets, secondhand smoke, or other airborne allergens. The company aims to provide flexible, comprehensive, and personalized health evaluations.

Requirements

Active, unrestricted license(s) in coverage area(s)
Board certification (required)
Able to cover the following primary locations: Suffolk and surrounding area
Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants.
