About The Position

Hartford HealthCare is seeking an experienced per diem Nurse Practitioner or Physician Assistant for our REACH (Resource for Elderly And Care at Home) Program serving our geriatric patients in the greater Bridgeport area. This role is open to Nurse Practitioners (FNP, ANP, AGNP) and Physician Assistants (PA) and is homecare based in the greater Bridgeport, CT area. The setting is Geriatric Medicine in the home, working within a multi-disciplinary care team. This is a per diem position, working weekdays. Hartford HealthCare supports Nurse Practitioners and Physician Assistants in practice with dedicated support and a voice through the Office of Advanced Practice. Requirements

• Unrestricted State of Connecticut Medical License (or eligible)
• APRN (Family, Adult/Geri Nurse Practitioner) or PA training
• At least two years of post-graduate experience as a Nurse Practitioner (APRN) or Physician Assistant (PA)

Responsibilities

• Perform routine physical examinations.
• Formulate medical, educational, and psychosocial plans to treat patients.
• Develop plans for further evaluation of health needs in consultation with physicians and care team members, including referrals to other health team members or specialists.
• Provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols.
• Identify problems outside of their scope of practice and refer appropriately to a physician.
