About The Position
Theoria Medical is seeking an APP (FNP, NP, or Adult Gero NP) or PA based in Tiffin, OH, to serve local skilled nursing facilities. The role focuses on building relationships, providing consistent care, and making a difference for vulnerable populations. Requirements
- Advanced Practice Providers must hold an active U.S. license
- Must be board certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants)
- Qualified specialties include but not limited to Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine

Responsibilities
- Provide high-quality, patient-centered care to residents in skilled nursing facilities
- Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues
- Communicate with facility nursing staff via phone or secure messaging as needed
- Maintain reliable on-site attendance for scheduled shifts
- Ensure compliance with clinical standards, facility policies, and regulatory requirements
