Full Time Nurse Practitioner or Physician Assistant - Fort Worth, TX
About The Position
Theoria Medical is seeking an APP (FNP, NP, or Adult Gero NP) or PA to serve local skilled nursing facilities within a close-knit, mission-driven care community. This role focuses on building trust and strengthening continuity of care for vulnerable populations. The position offers flexible scheduling and autonomy, supported by intuitive technology and a collaborative clinical team, allowing providers to focus on delivering exceptional care. Requirements
- Active U.S. license
- Board certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants)
- Qualified specialties include but not limited to Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
- Must be able to perform the essential functions of this position satisfactorily, with or without a reasonable accommodation.
- Theoria Medical conducts criminal background checks and pre-employment drug testing on all candidates upon acceptance of contingent offer.
Responsibilities
- Provide high-quality, patient-centered care to residents in skilled nursing facilities
- Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues
- Communicate with facility nursing staff via phone or secure messaging as needed
- Maintain reliable on-site attendance for scheduled shifts
- Ensure compliance with clinical standards, facility policies, and regulatory requirements
