About The Position

Mayo Clinic is seeking a Nurse Practitioner or Physician Assistant to join the Senior Services team in the Rochester region. This role involves working as part of a multidisciplinary team to provide care to individuals in private homes and senior living centers within the community. The NP/PA will collaborate with internal medicine physicians to deliver comprehensive care, including acute, chronic, preventive, and palliative services. Responsibilities include patient evaluations, ordering and interpreting diagnostic tests, prescribing medications and treatments, and patient counseling. Experience in Palliative Care, Primary Care, and/or Geriatrics is preferred.

Requirements

2+ years of APP experience.
Nurse Practitioner: Masters-prepared graduate of an accredited school of nursing and Nurse Practitioner program.
Nurse Practitioner: Current MN RN and APRN license and certification as a FNP, ANP or ACNP.
Physician Assistant: Graduate of accredited Physician Assistant program.
Physician Assistant: Current PA certification by NCCPA.
Physician Assistant: Registration as a PA by the MN State Board of Medical Practice.
Active BLS and ACLS certification from American Heart Association or American Red Cross at start date.
DEA registration required.
