About The Position

Theoria Medical is seeking an Advanced Practice Provider to serve local skilled nursing facilities within a close-knit, mission-driven care community. This role focuses on providing high-quality, patient-centered care to residents, including daily rounding, prescribing medications, addressing acute issues, and communicating with facility staff. The position emphasizes building trust and strengthening continuity of care for vulnerable populations, offering a unique blend of medical excellence and technological advancement.

Requirements

Active U.S. license
Board-certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants)
Qualified specialties include, but are not limited to, Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
Must be able to perform the essential functions of this position satisfactorily, with or without a reasonable accommodation.

Responsibilities

Provide high-quality, patient-centered care to residents in skilled nursing facilities
Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues
Communicate with facility nursing staff via phone or secure messaging as needed
Maintain reliable on-site attendance for scheduled shifts
Ensure compliance with clinical standards, facility policies, and regulatory requirements
