About The Position
Impact Health is seeking a compassionate and motivated Nurse Practitioner or Physician Assistant to join our growing clinical team on a Per Diem (PRN) basis serving patients within skilled nursing care facilities in West Hartford, CT. Impact Health PC is a leading advanced practitioner group in skilled nursing facilities across the Northeast, dedicated to prioritizing patient care. We specialize in Medical, Psychiatric, Wound Care and Palliative Care. This role offers a flexible Monday through Friday schedule with no nights, weekends, or holidays, providing an excellent opportunity for work-life balance and the ability to make a meaningful impact while delivering high-quality patient care.
Requirements
- Active license as a Nurse Practitioner (NP) or Physician Assistant (PA)
- Minimum 2 years of post-certification experience
- Proficient with EHRs and clinical technology
- Strong communicator with a collaborative mindset
Nice To Haves
- Experience in SNFs, hospitalist roles, emergency departments, or ICU preferred
Responsibilities
- Assess, develop and communicate comprehensive treatment plans using evidence based best practices
- Documentation withing 24-48 hours along with placing relevant orders for diagnostics and medications
- Monitor and manage acute and chronic conditions
- Compliance with facility, state and federal regulations/protocols.
- Support quality initiatives and process improvement plans.
- Collaborate with interdisciplinary team, nurse managers, physicians and specialists
- Educate and consul patients and families regarding course of treatment
