About The Position

The job is an integral part of the care delivery system, focused on providing a healing environment and placing customers first. The incumbent provides primary healthcare services for non-acute or chronic health conditions in specific populations, adhering to applicable scope and standards of practice, and the organization's policies, values, and mission within the CRNP scope of practice. Work is performed in a physician office or offices dedicated to Primary Care, including weekend and on-call coverage on a rotational basis, as required.

Requirements

  • Master's degree MSN from the practicing State Board of Nursing Approved Master's Program (for Nurse Practitioners)
  • National Board Certification for Nurse Practitioners through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), National Certification Corporation (NCC), or Pediatric Nursing Certification Board (PNCB) (for Nurse Practitioners)
  • Practicing-state licensure as a Nurse Practitioner (for Nurse Practitioners)
  • Prescriptive Authority from the practicing state (for Nurse Practitioners)
  • Master’s degree in Physician Assistant Studies (for Physician Assistants)
  • Current PA Physician Assistant License (for Physician Assistants)
  • PA-C or board eligible (for Physician Assistants)
  • Ongoing successful recertification (NCCPA) as a PA-C (for Physician Assistants)
  • Drug Enforcement Agency Licensure (within 60-90 days of hire)
  • CPR – American Heart Association (within 60 days of hire)
  • Act 34 Criminal Background Clearance Certificate
  • Act 33 Child Abuse Clearance Certificate
  • Act 73 FBI Fingerprinting Criminal Background Clearance Certificate
  • Four to ten years related experience and/or training; or equivalent combination of education and experience.

Responsibilities

  • Provides appropriate patient care to include recording patient medical history and physical assessment, developing therapeutic care plan, ordering diagnostic tests, interpreting test results, providing accurate medical record documentation.
  • Communicates and coordinates patient care with patient, family, community health services and interdisciplinary team. Involves attending physician regarding plan of care and involves physician in clinical decision making of any issues that are outside the CRNP's knowledge base or scope of practice.
  • Provides monitoring and continuity of care and coordinates periodic evaluation by physicians as defined in the written agreement between the CRNP and the supervising physician(s).
  • Prepares written prescription orders for drugs and controlled substances, administers medication and injections, sutures wounds and provides wound care
  • Educates patients and family members regarding health and preventive medicine, medications prescribed, treatment instructions and recommends community resources as appropriate.
  • Triages patient telephone calls, provides monitoring and continuity of care
  • Participates, develops, and implements performance and/or quality improvement initiatives. Ensures compliance with all regulatory agencies.
  • May perform other duties such as manage outpatient concerns and any other duty specific to specialty.
  • Other duties as assigned or requested.
