Nurse Practitioner or Physician Assistant (NP or PA) Palliative Care

The primary location will be Abbott Northwestern Hospital. Privileges will be obtained for all metro hospital sites to allow the incumbent the ability to float to different inpatient sites if needed. There is potential for occasional call or weekend work, with the current call rotation being none.
Requirements
- Master's degree - Completion of accredited Nurse Practitioner or Physician Assistant program
- Licensed Registered Nurse - MN Board of Nursing or equivalent Physician Assistant licensure required upon hire
- Certified Nurse Practitioner - American Nurse Credentialing Center (ANCC) certification or equivalent Physician Assistant licensure within specialty required upon hire
- ACLS-BLS Tier 2 - Multisource required within 180 Days
- Drug Enforcement Administration required upon hire
- Valid Driver’s License required upon hire - MN and WI residents must obtain a valid driver’s license in their state of residence within 60 days of employment for those working in Complex Care for Seniors, Community Palliative Care and Hospice
Nice To Haves
- Minimum 2 years' experience in palliative care, internal medicine, family practice, hospital medicine, or oncology
Responsibilities
- Provides clinical care for patients
- Completes comprehensive patient assessments.
- Determines diagnosis and treatment of illness.
- Orders and interprets medical diagnostics in tandem with attending physician.
- Prescribes medications, treatments, therapy and medical equipment.
- Conducts patient education when working to manage chronic illness or wellness care.
- Refers for specialty care when determined.
- Consults with physician when patient needs outside of APP scope of practice, diagnosis or treatment plan is unclear.
- Promotes communication within the team and other interdisciplinary health care staff.
- Acts as resource to members of the team.
- Works with primary care team for program development.
- Acts as liaison with outside agencies as needed.
- Collaborates with hospital staff by consulting and rounding on patients if designated by the specialty.
