About The Position

ChristianaCare's nationally recognized Swank Center for Memory Care and Geriatric Consultation is seeking an experienced and/or fellowship trained Nurse Practitioner or Physician Assistant to join our team as a full-time (1.0 FTE) virtual Memory Care Specialist Advanced Practice Clinician and Inboxologist. This unique position combines virtual patient care, clinical triage, caregiver support, care coordination, and population management to improve access for patients living with memory disorders and neurocognitive conditions. The ideal candidate is passionate about dementia care, enjoys working closely with families and caregivers, and thrives in a collaborative environment. As a Memory Care Specialist APC, you will serve as a dedicated clinical resource for patients and caregivers between scheduled visits, helping address urgent and emerging concerns, coordinating care, and supporting continuity across the patient journey. Through Epic inbox management, virtual care delivery, and interdisciplinary collaboration, you will help ensure patients and families receive timely, expert support when they need it most. This position also offers the opportunity to help shape and refine an innovative care model designed to improve access, responsiveness, and support for patients with cognitive disorders. Candidates with specialty or fellowship training are strongly encouraged to apply. This position exclusively supports patients of the Swank Center for Memory Care and Geriatric Consultation.

Requirements

  • Master's or Doctoral degree from an accredited Nurse Practitioner or Physician Assistant program.
  • Board Certification as one of the following: Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (AGNP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), Physician Assistant (PA-C), Psychiatric Mental Health Nurse Practitioner (PMHNP) with relevant experience.
  • Current Delaware APRN or PA license, or eligibility for Delaware licensure.
  • DEA registration and Delaware Controlled Substance Registration (CSR).
  • BLS certification.
  • Fellowship training, advanced specialty training, or a minimum of three years' clinical experience in geriatrics, dementia care, neurocognitive disorders, geriatric psychiatry, or a related specialty.
  • Strong clinical assessment, triage, care coordination, and decision-making skills.

Nice To Haves

  • Experience caring for patients with dementia, memory disorders, or other neurocognitive conditions.
  • Experience providing telehealth or virtual care services.
  • Experience with Epic and ambulatory inbox management workflows.
  • Experience supporting caregivers and families in an outpatient specialty setting.

Responsibilities

  • Manage patient, caregiver, and provider communications through Epic In Basket.
  • Evaluate and triage urgent and semi-urgent clinical concerns related to memory disorders and neurocognitive conditions.
  • Conduct virtual visits and provide follow-up care between scheduled appointments.
  • Review diagnostic results, manage medications, and coordinate treatment recommendations.
  • Educate and support patients and caregivers while facilitating access to community and specialty resources.
  • Collaborate with physicians, APCs, nurses, social workers, and other interdisciplinary team members.
  • Provide clinical support during provider absences and scheduling gaps.
  • Participate in initiatives that improve access, quality, workflows, and continuity of care within the Swank Center.
