This position will be in the Nephrology & Hypertension division, functioning as a NP/PA independent outpatient provider as well as hospital rounding on nephrology patients. The Nephrology & Hypertension NP/PA will work on a consulting service to provide 24/7 coverage managing chronic kidney disease (CKD), end stage renal disease (ESRD), acute kidney injury (AKI), hypertension, and acid base & electrolyte disturbances for patients throughout the hospital. In outpatient clinic the NP/PA will participate in an APP CKD clinic, outpatient dialysis rounds and collaborative specialty nephrology clinics. The Nephrology & Hypertension NP/PA will work collaboratively with the multidisciplinary team, including nephrologists, fellows, nurses, and others. The role requires timely assessments, interventions, interpretation of diagnostic tests, and management of acute and chronic medical conditions. Includes both Hospital Coverage (days, evenings, nights, weekends) and Ambulatory care. A minimum of 25% and up to 50% of coverage expected as in-house hospital coverage. Outpatient clinic visits, including rounding in ESRD outpatient dialysis unit. Work independently to the full scope of practice in compliance with state licensure. Perform and documents in the medical record detailed patient history and physical examinations. Coordinates daily rounds with the attending physician teams; presenting patient report to develop, collaborate and implement individualized plan of care. Follows assigned patient population throughout inpatient shift. Delivers patient education regarding kidney diseases, electrolyte derangements, CKD care and renal replacement therapies, including dialysis modality options. Consults with surgical teams to promote optimal dialysis access placements for patients. Coordinates care of ESRD patients with outpatient dialysis centers from admission to discharge. Prescribes appropriate pharmacological treatments and therapeutic procedures, including dialysis and continuous renal replacement therapies, as per accepted evidence-based guidelines and expected institutional protocols. Monitor effectiveness of therapeutic interventions. Participate in the delivery of Tele-Health for Advanced Care at Home Patient. Make appropriate referrals as needed. Examine plan of care for cost-effectiveness and achievement of expected outcomes. Assist with ongoing departmental process improvement and policy development. Actively attends and participates in educational lectures as part of the MD and APP fellowship program, grand rounds, quality improvement and faculty meetings. Precepts, mentors and instructs students, graduates, and novice NP/PAs.
