About The Position

ChristianaCare is seeking Nurse Practitioners or Physician Assistants to join ChristianaCare Hospitalist Partners (CCHP) in a nocturnist role. CCHP is a large, established hospital-employed group with over 150 physicians and 30 advanced practice clinicians serving three hospital campuses with 1,300 beds across Newark and Wilmington, DE, and Elkton, MD. The selected candidate will provide clinical management to patients at the Christiana Campus (Newark, DE) and remotely to patients on the Wilmington (Wilmington, DE) and Cecil (Elkton, MD) campuses, with support from a hospitalist physician. Full-time and part-time positions are available.

Requirements

  • Licensed or eligible for Nurse Practitioner or Physician Assistant license in the state of Delaware
  • Acute Care NP license required
  • Prescriptive Authority or eligibility for the state of Delaware
  • DEA registration required
  • ACLS required

Responsibilities

  • Performing admissions, cross coverage, and discharges on patients admitted under inpatient or observation status.
  • Assessing the health status of assigned patients through complete physical examination and medical history.
  • Analyzing patient data and formulating an individualized plan of care based on patient assessment and needs in collaboration with the physician, nursing, and other members of the multidisciplinary team.
  • Evaluating and monitoring the effectiveness of the prescribed management plan.
  • Establishing priorities based on patient needs and acting on them accordingly.
  • Ordering, interpreting, and/or performing necessary diagnostic testing or labs.
  • Initiating referral to appropriate resources and services.
  • Fostering efficient patient flow through the system or discharge.
  • Completing appropriate and timely documentation in the electronic health record (EHR), which includes history and physical (H&P), consults, progress notes, orders, and discharge summaries.
  • Submitting billing in a timely fashion according to policy.
  • Utilizing teaching/learning principles coupled with expert clinical knowledge to promote and provide education and counseling to the patient and/or family, including teach back.
  • Acting as a liaison for patients, families, and other health professionals.
  • Participating in discharge planning, resource allocation, and/or family meetings.
  • Contributing to the development and maintenance of best practice guidelines/protocols, quality and performance improvement projects, algorithms, and research.
  • Providing excellent customer service and assisting in efforts to improve patient satisfaction scores.
  • Striving for the highest quality in all aspects of work performance and assuming responsibility for improving care/services, including onboarding new team members.
  • Learning and using new information, skills, and knowledge.
  • Maintaining privileges, mandatory education, and regulatory requirements as needed for the area of specialization.
