About The Position

We are seeking a compassionate and skilled Nurse Practitioner or Physician Assistant to join our healthcare team, delivering high-quality primary care in the comfort of patients' homes and/or senior living facilities. In this role, you will provide direct patient care, focusing on promoting wellness, managing chronic conditions, and coordinating care with other healthcare providers. This position offers a rewarding opportunity to improve the health and quality of life for seniors within residential homes, as well as, assisted and independent living communities.

Requirements

  • Valid nurse practitioner or physician assistant license in state of operations & current board certification in your field
  • Master's Degree in Nursing or Physician Assistant Studies
  • Must possess excellent written and oral communication skills
  • Must possess traits of punctuality, organizational skills, and ability to function within a team.
  • This role requires travel to and from one or more assigned facilities.

Responsibilities

  • Perform comprehensive assessments, develop treatment plans, and provide ongoing care management for patients, focusing on preventive care and chronic disease management.
  • Conduct regular visits to assess patients’ conditions, adjust treatment plans, and address any emergent health concerns.
  • Collaborate with facility staff, family members, and other healthcare providers to ensure comprehensive, coordinated care tailored to the patients' needs.
  • Educate patients, families, and facility staff (when applicable) on disease prevention, wellness, and managing chronic illnesses to support better health outcomes.
  • Maintain accurate, timely, and thorough documentation within the electronic medical record (EMR) system, ensuring compliance with regulatory standards and organizational protocols.
  • Participate in quality improvement initiatives to enhance care delivery, reduce hospital readmissions, and optimize patient satisfaction within the facility and homes.
