About The Position

University of Minnesota Physicians is seeking a Certified Nurse Practitioner or Certified Physician Assistant to join our Cardiovascular Genetics practice at the M Health Fairview Clinics and Surgery Center in Minneapolis. This role involves providing comprehensive, longitudinal ambulatory care for adults with inherited or suspected genetic cardiovascular conditions. You will perform patient evaluations, develop individualized plans of care, and manage ongoing surveillance for patients with inherited cardiovascular disease. Collaboration with cardiovascular genetic counselors, physician specialists, and multidisciplinary teams is key. You will also participate in the review and interpretation of genetic testing results, incorporate family histories into risk assessment, and identify patients who may benefit from genetic evaluation. The position includes participation in multidisciplinary team meetings, communication with referring providers, and providing General Cardiology care with an emphasis on cardiac sarcoidosis. You will support patient access through independent clinics and collaborative practice, and contribute to the program's development, including clinical workflows and patient education. A commitment to exemplary patient care, evidence-based practice, and resource utilization is expected, with care individualized to patient needs. Occasional cross-coverage for inpatient General Cardiology may be required. Documentation and communication will be managed through an integrated Electronic Medical Record (Epic). Possible outreach sites include Maple Grove or Fridley.

Requirements

• Graduate of an accredited Physician Assistant or formal Adult Nurse Practitioner training program.
• Eligible or is certified through NCCPA, ANCC or AANP.
• Licensed or eligible to acquire a license and prescriptive authority through the MN Board of Medical Practice (PAs) or MN APRN license or eligible to acquire a license and prescriptive authority through MN Board of Nursing (NP’s).
• Able to meet threshold criteria/qualifications for credentialing and privileges.

Responsibilities

• Provide comprehensive, longitudinal ambulatory care for adults with inherited or suspected genetic cardiovascular conditions.
• Perform new and follow-up patient evaluations, including comprehensive histories, physical assessments, review of diagnostic testing, and development of individualized plans of care.
• Provide ongoing management and surveillance for patients with inherited cardiovascular disease.
• Coordinate appropriate cardiac imaging, rhythm monitoring, laboratory testing, and other diagnostic evaluations.
• Work closely with cardiovascular genetic counselors and physician specialists in the evaluation of patients and families.
• Participate in the review and interpretation of genetic testing results within the APP's scope of practice and in collaboration with genetic counselors and physicians.
• Incorporate genetic testing results into individualized cardiovascular management, surveillance plans, and the importance of appropriate screening for at-risk family members.
• Review and incorporate multigenerational family histories and pedigrees into cardiovascular risk assessment and clinical decision-making.
• Help identify patients who may benefit from genetic evaluation and facilitate appropriate referrals for genetic counseling and testing.
• Collaborate closely with cardiovascular genetics, genetic counseling, electrophysiology, heart failure, cardiovascular imaging, vascular/aortic specialists, and other multidisciplinary partners.
• Participate and help lead monthly Cardiovascular Genetics Multidisciplinary team meetings.
• Communicate recommendations and plans of care with referring physicians, primary care providers, and outside institutions.
• Provide General Cardiology care, including evaluation and longitudinal management of cardiovascular conditions with an emphasis on cardiac sarcoidosis.
• Support timely patient access through independent APP clinics and collaborative physician/APP practice.
• Participate in the continued development and growth of the Cardiovascular Genetics program, including opportunities to help develop clinical workflows, care pathways, patient education, and best practices for inherited cardiovascular disease.
• Demonstrate a commitment to exemplary patient care, evidence-based practice, and appropriate utilization of healthcare resources.
• Deliver care individualized to the developmental, cultural, and clinical needs of the population served.
• Provide occasional cross-coverage for inpatient General Cardiology based on clinical and operational needs.
• Utilize a fully integrated Electronic Medical Record (Epic) for documentation, communication, and coordination of care.
