University of Minnesota Physicians is seeking a Certified Nurse Practitioner or Certified Physician Assistant to join our Cardiovascular Genetics practice at the M Health Fairview Clinics and Surgery Center in Minneapolis. This role involves providing comprehensive, longitudinal ambulatory care for adults with inherited or suspected genetic cardiovascular conditions. You will perform patient evaluations, develop individualized plans of care, and manage ongoing surveillance for patients with inherited cardiovascular disease. Collaboration with cardiovascular genetic counselors, physician specialists, and multidisciplinary teams is key. You will also participate in the review and interpretation of genetic testing results, incorporate family histories into risk assessment, and identify patients who may benefit from genetic evaluation. The position includes participation in multidisciplinary team meetings, communication with referring providers, and providing General Cardiology care with an emphasis on cardiac sarcoidosis. You will support patient access through independent clinics and collaborative practice, and contribute to the program's development, including clinical workflows and patient education. A commitment to exemplary patient care, evidence-based practice, and resource utilization is expected, with care individualized to patient needs. Occasional cross-coverage for inpatient General Cardiology may be required. Documentation and communication will be managed through an integrated Electronic Medical Record (Epic). Possible outreach sites include Maple Grove or Fridley.
