About The Position

The Advanced Care at Home (ACH) program at Mayo Clinic Arizona is an innovative care model within the Division of Hospital Internal Medicine. It delivers comprehensive, hospital-level care to patients in their homes using advanced technology, virtual care, and in-person clinical services. The Nurse Practitioner/Physician Assistant (NP PA) is crucial in this model, providing safe, high-quality, patient-centered care across the continuum. They collaborate with physicians, nurses, pharmacists, case managers, command center operations, and community-based care teams to achieve exceptional clinical outcomes and patient experiences. As a key member of this evolving model, the NP PA acts as a clinical expert and ambassador for hospital-at-home medicine. This role involves championing the program, educating others about its benefits, and demonstrating enthusiasm for this innovative approach. Responsibilities

  • Independently and collaboratively manages patients receiving hospital-level care in the home setting.
  • Obtains comprehensive patient histories and performs physical examinations.
  • Formulates diagnostic impressions and develops evidence-based treatment plans.
  • Orders, interprets, and performs diagnostic and therapeutic procedures within scope of practice.
  • Monitors patient progress and evaluates the effectiveness of therapeutic interventions.
  • Coordinates consultations, referrals, and transitions of care.
  • Provides education, counseling, and support to patients and their families.
  • Collaborates closely with interdisciplinary care teams to ensure seamless, high-quality care delivery.
  • Maintains prescribing authority and clinical privileges as required by Arizona regulatory and institutional standards.
  • Serves as an advocate for quality, safety, service excellence, and continuous improvement within the ACH program.
  • Promotes awareness and understanding of the ACH model among patients, families, and healthcare colleagues.
