We're working with a healthcare team focused on managing complex Medicaid populations through a telehealth-first, coordinated care model , designed to improve outcomes while reducing unnecessary hospital utilization. Requirements

  • Nurse Practitioner license (AZ)
  • Master's degree or higher
  • Strong communication and care coordination skills
  • Have 3–5+ years of clinical experience
  • Are comfortable practicing independently
  • Have experience with telehealth or remote patient management
  • Enjoy working with complex populations (Medicaid, elderly, behavioral health)
  • Thrive in a startup or evolving environment
  • Want to have a voice in shaping care delivery models
