We're working with a healthcare team focused on managing complex Medicaid populations through a telehealth-first, coordinated care model , designed to improve outcomes while reducing unnecessary hospital utilization. Requirements
- Nurse Practitioner license (AZ)
- Master's degree or higher
- Strong communication and care coordination skills
- Have 3–5+ years of clinical experience
- Are comfortable practicing independently
- Have experience with telehealth or remote patient management
- Enjoy working with complex populations (Medicaid, elderly, behavioral health)
- Thrive in a startup or evolving environment
- Want to have a voice in shaping care delivery models
