About The Position

The Care Company is a home health care organization focused on providing compassionate, high-quality home health care services to clients of all ages, enabling them to live safely and well in their homes. The organization is seeking an experienced Nurse Practitioner (NP) to offer virtual clinical oversight and interim primary care support within a defined community care area. This role is designed to be consultative, virtual-first, and sustainable, providing meaningful clinical work without the typical pressures of a traditional clinic or permanent patient roster. The position involves supporting a small number of medically complex clients receiving structured home-based services. Most interactions are virtual, with in-person visits only required when clinically necessary and pre-approved. Each NP is assigned a single, manageable service area, typically a cluster of neighboring communities within Ontario Health at Home’s Central service area (e.g., York Region & North Toronto corridors). The role is not clinic-based and does not involve daily home visiting. For clients temporarily without a family physician, the Nurse Practitioner will provide interim, time-limited primary care oversight, including assessment, medication management, and clinical decision-making, until long-term primary care is secured. This is not an open-ended or indefinite rostered role, and care is delivered within a shared, interdisciplinary model, tied to the duration of the home-care episode.
