About The Position

This is a unique hybrid role for a Nurse Practitioner that blends the continuity of outpatient care with the fast-paced, collaborative environment of hospital medicine. The alternating-week schedule allows the practitioner to practice at the top of their license across the full continuum of care, spending one week in the outpatient clinic and the next managing inpatient care as a hospitalist. The role involves working alongside an experienced interdisciplinary team committed to delivering safe, compassionate, and evidence-based care. Requirements

  • Graduate of an accredited Nurse Practitioner program (MSN or DNP)
  • Active Nurse Practitioner license in the applicable state
  • National certification as an NP (ANCC, AANP, or equivalent)
  • Current DEA registration (or ability to obtain)
  • Current BLS certification; ACLS preferred or required for hospitalist role
  • Ability to obtain hospital privileges
  • Prior experience in outpatient clinic and inpatient/hospitalist settings
  • Background in Internal Medicine, Family Medicine, or Hospital Medicine
  • EMR proficiency

Nice To Haves

  • Experience in rural healthcare or Critical Access Hospital settings preferred
  • ACLS preferred or required for hospitalist role

Responsibilities

  • Conduct comprehensive patient evaluations, including medical histories, physical exams, and diagnostic assessments.
  • Diagnose, treat and manage acute and chronic medical conditions within the scope of practice.
  • Develop and implement individualized, evidence-based treatment plans.
  • Prescribe medications and therapies.
  • Order, perform and interpret laboratory tests, imaging studies, and other diagnostics.
  • Provide preventive care, wellness counseling, immunizations, and health screenings.
  • Deliver compassionate patient and family education on disease prevention and management, medication adherence, and healthy lifestyle choices.
  • Collaborate with specialists & ancillary services and coordinate referrals as needed.
  • Maintain timely, accurate and compliant EMR documentation.
  • Manage hospitalized patients through admissions, daily rounding, and discharges.
  • Perform history, physical examinations and develop care plans for admitted patients.
  • Monitor patient progress and respond to changes in clinical condition.
  • Collaborate with physicians, nursing staff, therapy services, consultants, and case management teams.
  • Participate in interdisciplinary rounds and care conferences.
  • Support safe discharge planning, follow up care coordination and transitions of care.
  • Communicate clearly with patients, families, outpatient providers and care teams.
  • Complete admission notes, progress notes, and discharge summaries and required documentation in the EMR.
