Local travel is an essential part of this role, with approximately 75% travel to support patient care needs and collaborate with healthcare partners throughout the community.

Requirements

  • Current and unrestricted Nurse Practitioner license.
  • Active DEA and CDS licenses.
  • Current BLS and ACLS certifications.
  • Valid driver's license and reliable transportation.
  • Willingness and ability to travel regularly throughout the service area.

  • Experience practicing as a n Advanced Practice Nurse in clinical settings such as primary care, hospitals, outpatient clinics, emergency medicine, or related healthcare environments .

  • Strong clinical assessment and diagnostic skills.
  • Knowledge of healthcare regulations, quality standards, and patient care best practices.
  • Exceptional communication and patient education abilities.

Responsibilities

  • Perform comprehensive patient assessments, physical examinations, and medical history reviews.
  • Order, interpret, and evaluate diagnostic testing, including laboratory studies and imaging.
  • Diagnose and treat acute and chronic vascular conditions.
  • Develop, implement, and manage individualized treatment plans.
  • Prescribe medications and monitor treatment effectiveness.
  • Educate patients and caregivers on health conditions, preventive care, and treatment plans.
  • Collaborate with referring providers and interdisciplinary care teams to ensure seamless patient care.
  • Maintain accurate and timely clinical documentation.
  • Monitor patient progress and adjust care plans as needed to achieve optimal health outcomes.
