About The Position

Innovative Wound Solutions, a leading healthcare organization dedicated to providing innovative and patient-centered care, is seeking a highly motivated and experienced Nurse Practitioner to work within our newly established Wound Care Program. Requirements

• Current and unrestricted Nurse Practitioner license in UT.
• Minimum of 1 year of experience in wound care.
• Strong interpersonal and communication skills.

Nice To Haves

• Certification as a Nurse Practitioner with a specialization in wound care management is preferred.

Responsibilities

• Provide direct patient care through all available channels and products.
• Conduct comprehensive assessments, develop treatment plans, and implement evidence-based care practices.
• Monitor and evaluate patient progress, adjusting treatment plans as needed.
• Collaborate with interdisciplinary teams to ensure seamless patient care coordination.
• Develop personalized treatment plans for patients with wounds, focusing on long-term management and prevention of complications.
• Educate patients and families on self-management strategies and lifestyle modifications.
• Establish and maintain effective communication channels with referring providers to ensure continuity of care.
• Collaborate with healthcare professionals, including physicians, nurses, and other specialists, to optimize patient care.
