About The Position
CareOne in partnership with BridgeCare Provider Group is Now Hiring - Nurse Practitioner - Internal Medicine. We are seeking a compassionate and experienced Nurse Practitioner (NP) to provide comprehensive medical care to residents in a skilled nursing facility (SNF). The NP will evaluate and manage acute and chronic medical conditions, develop individualized treatment plans, coordinate care with the interdisciplinary team, and promote quality outcomes for a medically complex, primarily older adult population. Travel will be required - no overnight travel necessary for training.
Requirements
- Must have a Nurse Practitioner's license in the state of operations
- Must have 2+ years' experience working within Family Practice, General Practice, Geriatric care or related field,
- Must have a valid driver's license and a reliable source of transportation
- Must be willing to travel

- Experience in a nursing home, hospital, assisted living or acute rehab space is strongly PREFERRED

Responsibilities
- Conduct visits to patients in accordance with local, state and federal regulations.
- Communicate visit findings to designated members of the interdisciplinary team.
- Facilitate access to appropriate support models of care (i.e., rehabilitation, on-going aggressive treatment).
- Consult with other members of interdisciplinary team to contribute to the patient's plan of care.
- Prepare and maintain accurate patient records, charts and documents.
- Assist in development of clinical practice guidelines/standards in support of quality care as needed.
- Ensure collaboration with patients, caregivers, medical directors and members of IDT.
