As a Nurse Practitioner in our Infant High-Risk Clinic, you will play a critical role in the longitudinal care of medically complex infants transitioning from the NICU to home. This position focuses on providing comprehensive follow-up care to patients from NICU discharge through approximately 2–3 years of age. You will work within a highly collaborative, multidisciplinary team dedicated to optimizing developmental outcomes, growth, and family support. You will care for infants and young children with complex medical and developmental needs, including those with: Birth gestational age <33 weeks Discharge with feeding support (NG-tube or G-tube) Oxygen dependence at discharge Hypoxic-Ischemic Encephalopathy (HIE) Congenital Diaphragmatic Hernia (CDH) In utero substance exposure Developmental delays or concerns Other conditions requiring close nutritional and developmental follow-up
