The Nurse Practitioner (NP) specializing in house calls provides high-quality, patient-centered care to individuals in their homes, particularly focusing on those with chronic illnesses, mobility issues, or limited access to healthcare facilities. This role emphasizes holistic care, patient education, and the management of complex medical conditions.

Requirements

  • Master's Degree in a Nurse Practitioner Program: Family or Adult Geriatrics
  • Current New York State License as a Nurse Practitioner.
  • National Board Certification require American Association of Nurse Practitioners (AANP) American Nurses Credentialing Center (ANCC) Oncology Nursing Certification Corporation (ONCC)

Nice To Haves

  • Bilingual (Spanish Speaking) is a plus

Responsibilities

  • Conduct comprehensive assessments of patients’ health status, including physical examinations, medical history reviews, and medication reconciliations during home visits.
  • Diagnose medical conditions, develop and implement individualized treatment plans, and prescribe medications as needed.
  • Collaborate with physicians, specialists, and other healthcare providers to ensure continuity of care and comprehensive support for patients.
  • Provide education to patients and their families regarding health management, lifestyle modifications, disease prevention, and medication adherence.
  • Maintain accurate and thorough documentation of all patient interactions, including assessments, treatments, and follow-up care in electronic health records.
  • Schedule and conduct follow-up visits to monitor patient progress and adjust treatment plans as necessary.
  • Assess and manage urgent medical issues during home visits, providing referrals or interventions as needed.
  • Participate in outreach efforts to promote health services and educate the community on available resources.
