Intention Healthcare is seeking a compassionate and skilled Nurse Practitioner (NP) to provide high-quality, patient-centered care through home visits in Brooklyn and Queens boroughs of NYC. This role focuses on individuals with chronic illnesses, mobility challenges, or limited access to healthcare facilities. The NP will emphasize holistic care, patient education, and management of complex medical conditions, ensuring continuity of care for patients in their homes.

Requirements

  • Master’s Degree in a Nurse Practitioner program (Family or Adult Geriatrics).
  • Current NYS Nurse Practitioner license required.
  • National Board Certification through one of the following: American Association of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or Oncology Nursing Certification Corporation (ONCC).
  • Bilingual (Spanish-speaking) is a plus.
  • Experience with EMR/EHR systems preferred.
  • New graduates encouraged to apply.

Responsibilities

  • Conduct comprehensive health assessments during home visits, including physical exams, medication reconciliation, and review of medical histories.
  • Diagnose medical conditions, develop individualized care plans, and prescribe medications as appropriate.
  • Collaborate with physicians, specialists, and other healthcare professionals to ensure integrated and continuous care.
  • Educate patients and families on disease management, lifestyle changes, medication adherence, and preventive care.
  • Maintain detailed and accurate electronic health records (EHR) of patient assessments, treatments, and follow-ups.
  • Monitor patient progress through follow-up visits and adjust treatment plans as necessary.
  • Assess and manage urgent medical situations during home visits, providing timely interventions or referrals.
  • Participate in local outreach initiatives to promote health services and educate the community about available resources.
