The Nurse Practitioner will provide comprehensive, patient-centered care to individuals with serious illnesses. Based on market needs, the role may include caring for patients in hospice, palliative care, or both. The Nurse Practitioner will deliver advanced clinical care, perform patient management visits and hospice Face-to-Face (F2F) encounters for eligibility certification and recertification, and partner closely with physicians and interdisciplinary teams to ensure high-quality, compliant, and compassionate care. This role blends autonomy with collaboration, allowing you to guide patients and families through complex medical decision-making, symptom management, and end-of-life care. Compensation may vary within the salary range provided based on several factors including but not limited to a candidate’s location, experience, education, skills, licensure, certifications and department equity.
