Nurse Practitioner-Hospice/Palliative Care-Northern NM-CDS
About The Position
Presbyterian Medical Group seeks an outpatient focused Palliative Care Nurse Practitioner or Physician Assistant to join their established practice in Santa Fe, New Mexico. This role serves as a consultant in Palliative Medicine in both inpatient and outpatient settings, and as a hospice provider offering clinical oversight. The position involves assisting the Hospice Medical Director with documentation for certifications to obtain hospice benefits for patients. The practice includes an average of six home visits per day, with call taken in rotation with the whole team, one week at a time. Some hospice care will be a part of this role.
Requirements
- Masters degree in Nursing
- New Mexico Nurse Practitioner license
- Two (2) years working experience as a nurse practitioner
- AANC or AANP certification
- Certified Physician Assistant
- Two (2) years working experience as a Certified Physician Assistant
- Licensed by the New Mexico Board of Medical Examiners
Responsibilities
- Collaborates with colleagues in the management of patients, in outpatient settings
- Offers care to patients with critical and life-limiting illness, focused on assisting them to achieve medical treatment concordant with their goals.
- Offers expert symptom management to assist patients in achieving maximal quality of life, despite their illness.
- Builds effective relationships with referring physicians and care teams, strives to improve collaboration centered on the care of these patients.
- In both Palliative and Hospice settings, works with an interdisciplinary team, offering support to both patients and families.
- Works with colleagues to improve the quality of care offered patients
