Requirements

Graduation from an accredited Nurse Practitioner program.
Current Board Certification as a Nurse Practitioner.
Minimum of 2-3 years of home health care or home visit experience.

Nice To Haves

Bilingual in Armenian is a plus.

Responsibilities

Conduct 5-7 home visits daily for patients with complex care needs.
Perform comprehensive health evaluations, including detailed medical history and physical examinations, medication reviews and safety assessments, and evaluation of home safety and psychosocial needs.
Accurately document all pertinent patient information, including medications, end-of-life preferences, and factors contributing to preventable hospital admissions on the same day as visits.
Educate patients and their families to contact you before seeking medical care (except in emergencies).
Maintain patient safety and confidentiality while adhering to universal precautions in all care activities.
Prescribe treatments and medications within your scope of practice.
Rotate with the inpatient hospitalist team as determined by the Campus Medical Director to enhance clinical skills.
Evaluate patients at the Post Discharge Clinic alongside the attending physician.
Foster trust and strong relationships with patients and their families.
