This role is for a Nurse Practitioner certified under section 6910 of the New York State Education Law, providing primary health care in accordance with written policies approved by nursing/medical administration and responsible for patient care as outlined within the Collaborative Practice Agreement. The position is full-time, with shifts from 8 AM - 6 PM, Monday - Friday, involving 4-10 hour shifts.

Requirements

Completion of an accredited Adult-Gerontology Acute Care Nurse Practitioner, Family Nurse Practitioner, or Adult-Gerontology Primary Care Nurse Practitioner program
Current NY State NP license and registration
Valid DEA number
ANCC/AANP Certification
Experience in complex patient management as a Nurse Practitioner
Excellent communication and interpersonal skills focused on patient-centered care
Chemotherapy & Immunotherapy / Biotherapy Certification
1-2 years of Outpatient Clinical Oncology/Chemotherapy Patient Management experience
Proficient in ordering Chemotherapeutic & Immunotherapeutic regimen
Demonstrate expertise in infusion protocols
Proficient with EMR/Office/Microsoft Outlook applications

Nice To Haves

Skilled in managing complex oncology patient cases
Skills in providing comprehensive education and support to Oncology patients/families
Certification as an Advanced Oncology Certified Nurse Practitioner (AOCNP)

Responsibilities

Provides age-appropriate care and intervention for patients served (e.g., assessments, treatment plans, assists with assessments).
Provides support services using age-appropriate interpersonal communication.
Develops and implements a collaborative practice agreement in the specific clinical area and performs a periodic review.
Assesses the health status of clients/families through comprehensive health history and physical examinations.
Initiates therapeutic and diagnostic procedures, consultations, and laboratory examinations.
Interprets findings and alters plan of care, including prescribing and altering medication regimens and ordering blood according to the collaborative practice agreement and designated protocols.
Provides teaching and counseling in areas of health promotion, maintenance, and disease prevention.
Documents findings and plan of care in the patient’s medical record.
Consults with physician as established in the collaborative practice agreement.
Initiates referrals to specialty services and fosters continuity of care.
Initiates hospital admission and discharge after collaborative consultation with physician.
Works collaboratively with the interdisciplinary team to promote optimal patient outcomes.
Delivers assessment, treatment, and patient care responsibilities with knowledge of patient growth and development appropriate to the ages of patients served.
Demonstrates accountability for nursing practice.
Advocates for patients and supports patient rights.
Serves as a resource person to nurses and other health team members.
Participates in peer review.
Participates in approved medical and nursing research studies.
Participates in defining, maintaining, and interpreting standards of nursing practice.
Contributes to the maintenance of a positive image of professional nursing.
Participates in departmental and interdisciplinary committees that influence policies affecting nursing practice.
Performs duties and responsibilities in accordance with the agreement between Nurse Practitioner and collaborating physician.
Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental, and infection control standards.
Keeps knowledge current through self-directed learning and attendance at educational programs.
Performs other related duties as required.
