About The Position

This role involves conducting patient visits and assessments, managing hematology and oncology patients, ordering and monitoring therapies, providing patient and family education, and assisting with patient triage and acute care needs. The Nurse Practitioner will work collaboratively with supervising physicians to develop and implement evidence-based plans of care, ensuring timely patient access, thorough documentation, and appropriate follow-up care. Responsibilities include evaluating patients throughout the continuum of care, monitoring disease progression and treatment effectiveness, identifying and addressing treatment-related toxicities, and providing symptom management for various cancer-related concerns. The role also requires educating patients and caregivers, assessing their understanding, and assisting with the triage of patient concerns and urgent clinical issues to determine the appropriate level of care.

Requirements

  • Completion of an accredited Nurse Practitioner program.
  • Current state RN and NP licensure.
  • Board certification as a Nurse Practitioner.
  • Experience in Hematology/Oncology preferred.
  • Strong clinical assessment and diagnostic skills.
  • Proficiency in ordering and interpreting diagnostic tests and laboratory studies.
  • Knowledge of chemotherapy, immunotherapy, and targeted therapy.
  • Experience with symptom management for cancer-related concerns.
  • Excellent patient and family education skills.
  • Ability to collaborate effectively with physicians and healthcare team members.
  • Proficiency in electronic health record (EHR) systems.

Nice To Haves

  • Minimum of 2 years of experience as a Nurse Practitioner in Hematology/Oncology.
  • Experience with survivorship care planning.
  • Familiarity with specific institutional policies and collaborative practice agreements.

Responsibilities

  • Conducts comprehensive patient evaluations, including new patient consultations, follow-up visits, treatment assessments, and survivorship visits.
  • Obtains and documents complete medical histories, physical examinations, treatment responses, and symptom assessments.
  • Develops and implements evidence-based plans of care in collaboration with supervising physicians.
  • Ensures timely patient access, thorough documentation, and appropriate follow-up care.
  • Evaluates and manages patients with hematologic and oncologic diagnoses throughout the continuum of care.
  • Monitors disease progression, treatment effectiveness, laboratory results, imaging studies, and adverse effects.
  • Identifies and addresses treatment-related toxicities and complications in accordance with established protocols.
  • Collaborates with physicians regarding complex clinical issues and treatment modifications.
  • Orders and interprets diagnostic tests, laboratory studies, and imaging as clinically indicated.
  • Prescribes medications and supportive care interventions in accordance with state regulations, institutional policies, and collaborative practice agreements.
  • Monitors patient response to chemotherapy, immunotherapy, targeted therapy, and supportive medications.
  • Provides symptom management for pain, nausea, fatigue, anemia, infection risk, and other cancer-related concerns.
  • Educates patients and caregivers regarding diagnoses, treatment plans, potential side effects, symptom management, and survivorship care.
  • Reinforces treatment adherence and promotes informed decision-making.
  • Assesses patient understanding and identifies barriers to care.
  • Participates in the assessment and triage of patient concerns, treatment-related symptoms, and urgent clinical issues.
  • Determines appropriate interventions and level of care, including outpatient management, emergency evaluation, or hospitalization when necessary.
  • Supports efficient clinic operations while maintaining high standards of patient safety and quality care.
