Nurse Practitioner - Skilled Nursing Facility - New Grads Welcome
About The Position
This organization is dedicated to delivering convenient, high-quality primary care to geriatric patients in the comfort of wherever they call home. They currently boast a team of more than 30 physicians and nurse practitioners who ensure that patients receive the best medical care possible without the challenges of traveling to a provider’s office. Their services include, among others, primary care, psychiatric care, post-acute care, and transitional care. The nurse practitioner will serve as the primary care provider for geriatric patients located within a single skilled nursing facility (SNF) in Mentor. Providers see a variable number of patients per day, dependent upon patient needs. The practice uses a mobile-based EHR with AI functionality to reduce administrative time. On-call is phone triage only and providers rotate the responsibility evenly, with some flexibility in scheduling available.
Requirements
- Ohio NP license
- DEA
- No experience required

Responsibilities
- Conduct health assessments to identify patient needs
- Develop personalized care plans
- Regularly evaluate patient conditions and responses to treatment
- Adjust treatment plans as necessary
- Perform minor procedures and wound care as needed
- Educate patients and families on health management, disease prevention, etc.
- Provide guidance on lifestyle modifications and self-care practices
- Work with physicians, nurses, and other professionals to coordinate care
