About The Position

Vera Whole Health seeks an experienced provider who sets the standard of excellence as a clinical services leader within one of our primary-care clinics. Our model is built so practitioners can focus on patient care first, develop meaningful patient-provider relationships, and use empathetic listening to empower patients to actively participate in their wellness goals. Vera care centers are an outcomes-based model; the type of patients you see will depend on your skill set and may include patients from birth through seniors in Medicare Advantage programs. You will provide evidence-based primary care and partner closely with an on-site care team. Expect to meet with 10 to 15 patients each day for 20-to-60-minute appointments, supported by a Vera Whole Health Coach and other team members, delivering whole person care that addresses prevention, chronic management, acute needs, and disease/case management. There is convenience of an onsite lab and pharmacy to support diagnostic testing and treatment.

Requirements

  • NP Board Certification with primary care, family medicine, or internal medicine background
  • Current, valid and unrestricted ARNP license and valid DEA number
  • Spanish/English bilingual proficiency is an essential job function
  • Minimum 3–5 years of primary care experience
  • Minimum 2+ years with case management emphasizing health prevention and population health management
  • Documentation of proof of a negative TB test in the last 12 months before start date

  • Ability to collect comprehensive health histories, perform appropriate physical examinations, conduct diagnostic testing, and prescribe medications in alignment with the Vera medical home model

Responsibilities

  • Provide clinical leadership by participating in team meetings and committees, supporting process improvement and change management, and rotating with other providers in the region to ensure after-hours coverage.
  • Assess patient population needs, driving participation in annual whole health evaluations and health coaching, and meeting established benchmarks for clinical outcomes, engagement, and satisfaction.
  • Provide evidence-based primary care and partner closely with an on-site care team.
  • Meet with 10 to 15 patients each day for 20-to-60-minute appointments.
  • Deliver whole person care that addresses prevention, chronic management, acute needs, and disease/case management.
  • Support diagnostic testing and treatment with onsite lab and pharmacy.
