About The Position

The Nurse Practitioner Fellow is a post-graduate clinical trainee participating in the SNAPT Fellowship Program who provides mentored clinical care to patients through an interdisciplinary and collaborative approach that considers the psychosocial aspects of health and disease. The Fellow will transition after 12 months to traditional practice and is committed to a second year of employment in a Nurse Practitioner role with opportunities for ongoing employment.

Requirements

Ability to work with a multidisciplinary clinical team
Computer skills including Microsoft office suite, electronic medical records and practice management system
Recent graduate (within 2 years) of an accredited Nurse Practitioner program
Primary Care Adult or Family certification, or eligibility for certification prior to start of Fellowship
Maryland licensure and clinical certification, eligibility for DEA and Maryland CDS
Interest in and/or experience in community health settings
Interest in and/or experience in gender affirming care, HIV medicine, sexual health
Experience working in a diverse environment
Ability to work harmoniously with diverse groups of individuals required
Excellent communication and team skills required.
Must meet FTCA requirements.

Uses diagnostic testing appropriately yet cost-effectively, and providing appropriate treatment
Self-assessment and reflection on individual learning objectives, competencies, and goals
Effectively engages patients in their care
Maintains complete and comprehensive documentation
Participates in evaluation and feedback processes with preceptors and mentors
Seeks physician consultation when appropriate
Demonstrates professionalism and integrity in all aspects of patient care
Shows meaningful progress toward professional role development
Shows meaningful progress toward quality improvement goals
