About The Position

Central City Health (CCH) is seeking a Nurse Practitioner - Family to join their integrated healthcare team. CCH has been serving the under-housed and at-risk population in metro Detroit since 1972, providing a wide range of services including primary and pediatric care, dental care, behavioral and SUD care, and supportive housing. The Nurse Practitioner will be an active member of the integrated care team, contributing to the mission of achieving wellness in the community by providing an array of primary and behavioral health care, housing, and substance abuse services with dignity and respect. CCH is expanding its programming and offers opportunities for participation in the NHSC Loan Repayment Program and the Public Service Loan Forgiveness Program (PSLF).

Requirements

• Master’s degree in nursing required.
• Valid Michigan licensure as a Certified Nurse Practitioner required.
• Basic Life Support certification required.
• Must be able to work cooperatively with staff and others involved in the delivery of service to patients.
• DEA License (Preferred).
• MI Nurse Practitioner licensure (Required).
• Basic Life Support (BLS) Provider Certification (Required).

Nice To Haves

• 1 to 2 years of nursing experience in a FQHC, community mental health, public health or primary care setting preferred.
• Experience working with patients exhibiting severe and persistent mental illness preferred.
• ACLS preferred.

Responsibilities

• Provide peer support services to clients experiencing mental illness, chronic health condition(s) and/or homelessness.
• Function as a role model to peers, exhibiting competency in personal recovery and the use of coping skills.
• Serve as a patient advocate and provide information and support.
• Assist patients in regaining independence within the community and mastery over their own recovery process.
• Assist consumers/patients in effectively utilizing resources, managing symptoms of their illness, and developing daily living skills to gain greater independence through skill building and supportive interventions.
