SUMMARY: The Nurse Practitioner or Physician Assistant is accountable to facilitate a quality hospital experience and outcome for patients within a cost-effective time frame. The Nurse Practitioner or Physician Assistant will be responsible to the Medical Director of Cardiology and responsible for overseeing the progress of a specific caseload of patients across the care continuum, primarily for acute, in hospital care. Acts as an educator, consultant, and liaison to healthcare providers caring for the patients across the care continuum. The core Success Factors include: Instill Trust and Value Differences Patient and Community Focus and Collaborate RESPONSIBILITIES: Demonstrates a fundamental medical knowledge base with mastery of knowledge within a defined content area (cardiology).Able to manage a caseload of patients in partnership with the healthcare team to optimize health status and coordinate care.Provides leadership in the development, validation, implementation, and evaluation of standards, policies, procedures, and critical paths to support the patient care delivery model.Oversee care coordination in the acute care setting, including identifying the need for admission via triage from the Emergency Department, inpatient care, and discharge planning.Partner with the healthcare team to achieve mutually agreed upon healthcare goals by providing necessary support, education and encouragement.Demonstrates advanced physical and psychosocial assessment skills and knowledge of family dynamics.Provides direct care to patients in the acute care setting in collaboration with the Cardiologist.Performs advanced practice skills under the supervision of the Medical Director.Participates in the education, clinical research and mentoring of healthcare providers.Facilitates patient/family education in area of expertise.Position may include rotation to the Cardiology Procedural Care Unit and/or Advanced Heart Failure service.Annually completes mandatory safety infection control, BLS, and ACLS.Prescriptive privileges per licensure.
