About The Position

Greater Seacoast Community Health is seeking a compassionate and collaborative Nurse Practitioner (APRN) or Physician Associate (PA) to join our mission-driven Primary Care team in Portsmouth, NH. Requirements

Current NH RN and APRN licensure (for APRN candidates)
Board Certification through ANCC and/or AANP (for APRN candidates)
Minimum of 3–5 years of experience practicing as a Nurse Practitioner (for APRN candidates)
Current NH Physician Associate license (for PA candidates)
NCCPA Certification (for PA candidates)
Minimum of 3–5 years of experience practicing as a Physician Associate (for PA candidates)
Current DEA registration
Current BLS certification

Nice To Haves

ACLS/PALS preferred
Experience in Primary Care
Experience in Community Health
Experience in Family Medicine
Experience in FQHC environments is highly valued

Responsibilities

Provide comprehensive primary care services across the lifespan
Perform preventive health exams, diagnosis, treatment, and follow-up care
Manage acute and chronic medical conditions
Coordinate care and referrals with internal and external providers
Collaborate closely with multidisciplinary healthcare teams
Document patient care accurately within the EHR system
Participate in provider meetings, quality initiatives, and clinical collaboration activities
Deliver care with professionalism, compassion, and respect
