About The Position

This role is for a caring and dedicated professional who has completed an accredited PNP/PA program that includes clinical experiences in all aspects of primary healthcare across all age groups. At least one year of post-degree experience is preferred but not required. The ideal candidate will possess a Master’s or Doctoral degree in Nursing from an accredited Nurse Practitioner program with current licensure as a Registered Nurse and Advanced Practice Registered Nurse (APRN) in the state of Ohio and national certification as a Family Nurse Practitioner (FNP), OR current licensure as a Physician Assistant (PA) in the state of Ohio and current national certification as a Physician Assistant. The role requires skill and experience in patient assessment and treatment, working with multi-disciplinary teams, and demonstrating the ability to work with diverse populations with cultural competence. Regular use of various computer applications, including Microsoft Office, HRIS, EHR, and other online systems, is expected. The candidate must be able to obtain a Driver's License, maintain auto insurance, earn CPR/First Aid certification, and be willing to submit to a background check and fingerprinting.

Requirements

Completion of accredited PNP/PA program that includes clinical experiences in all aspects of primary healthcare across all age groups.
Master’s or Doctoral degree in Nursing from an accredited Nurse Practitioner program OR current licensure as a Physician Assistant (PA) in the state of Ohio.
Current licensure as a Registered Nurse and Advanced Practice Registered Nurse (APRN) in the state of Ohio AND national certification as a Family Nurse Practitioner (FNP) OR current national certification as a Physician Assistant.
Skill and experience in assessment and treatment of patients.
Ability to work with multi-disciplinary teams.
Ability to work with diverse populations as well as cultural competence.
Regularly use various computer apps including Microsoft Office, our HRIS, EHR, and other online systems, with multiple logins often simultaneously.
Ability to obtain a Driver's License.
Maintain auto insurance.
Earn CPR/First Aid certification.
Willing to submit to background check and fingerprinting.

Responsibilities

Practice within a Team Based Model, ensuring integrated care that addresses primary and behavioral health conditions through direct partnership with all disciplines involved in the treatment of clients.
Provide direct care services including review of health assessment forms, completion of comprehensive mental health evaluations, establishing diagnoses, determining appropriate treatment plans, provide ongoing care and making appropriate referrals.
Provide comprehensive primary care services, including health assessments, physical examinations, diagnosis, and treatment of acute and chronic illnesses.
Develop and implement individualized treatment plans based on patient needs and evidence-based practice.
Order, perform, and interpret diagnostic and laboratory tests.
Prescribe medications, treatments, and therapeutic interventions within the established scope of practice.
Provide preventive healthcare services, including routine screenings, immunizations, and health risk assessments.
Educate patients and families regarding disease prevention, health maintenance, medication management, and healthy lifestyle choices.
Coordinate and follow up on referrals to specialists, diagnostic services, and community resources as appropriate.
