About The Position

Community Research Foundation (CRF) is seeking a Nurse Practitioner for their Adult IMPACT program in San Diego, CA. The IMPACT program serves individuals experiencing homelessness or at-risk of homelessness who are diagnosed with serious mental illness and co-occurring conditions. Services are delivered in a trauma-informed and culturally sensitive manner, utilizing a wraparound approach for children and a psychosocial rehabilitation and recovery approach for adults. The program emphasizes an integrated approach to address both psychiatric and substance use conditions, considering the strengths, needs, abilities, and preferences of clients and their families. More than 75% of services are delivered in the community, and all clients are provided housing within the first week of enrollment. The Adult Services at IMPACT requires 10-20 hours of coverage per week, with a caseload of approximately 150 per FTE. Dedicated front office staff, translators, and clinicians support Psychiatrists to focus on medical management and patient care.
